RTM Global Workflow and its Contents

Overview

Part of the Keet RTM Solution is the RTM workflow that your clinic has access to. The RTM global workflow will automatically assign Questionnaires and pieces of Education to your patients at a regular cadence for 8 weeks after the provider adds an internal facing Questionnaire titled "RTM: Include in Patient's Care Plan" and answers "Yes". When a Questionnaire or piece of Education is assigned to the patient’s Care Plan, the patient will receive a notification on their mobile device, if they have enabled notifications from Keet in their mobile device’s settings. 

Purpose

The purpose of the RTM workflow is to encourage patient engagement with the Keet app, in order to increase the likelihood of satisfying the patient engagement requirement of the 98975, 98976 and 98977 codes. Read more about the billing requirements of each RTM code here.

RTM Global Workflow

Once your clinic has purchased the Keet RTM product, staff members with an Admin role associated with their Keet profile will be able to turn on the Keet RTM workflow. If necessary or desired, the RTM workflow can also be turned off by users with an Admin role in Keet.

Once the RTM workflow is turned on, the Keet system will automatically assign Questionnaires and pieces of Education to your patients at a predetermined cadence, starting when the provider adds an internal facing Questionnaire titled "RTM: Include in Patient's Care Plan" and answers "Yes". The RTM workflow will assign Questionnaires and pieces of Education to patients who have this  Questionnaire completed in their profile AFTER the RTM Global Workflow is turned on. If this Questionnaire is completed prior to the workflow being turned on, the patient will not be assigned RTM Questionnaires or pieces of Education. You can manually assign RTM Questionnaires or pieces of Education to a patient’s Care Plan, if desired. All RTM Questionnaires and pieces of Education are available in the Keet Library.

Questionnaires and Education

The Questionnaires and pieces of Education that are assigned as a result of the RTM workflows are custom to the Keet RTM product. They are designed to encourage patient compliance with assigned HEP, and to encourage communication between patients and their providers between visits. 

To Trigger the RTM Global Workflow you will need to manually add the Questionnaire titled “RTM: Include in Patient Care Plan” to the desired Care Plan for that patient and answer “Yes” to start the Workflow. This method was utilized so that providers can have more control over which patients receive the Workflow and content. If this Questionnaire is not assigned to a patient or if “No” is selected in this Questionnaire the patient will not be included in the RTM Global Workflow.

 

RTM Questionnaires

Patients will be assigned the following Questionnaires at regular intervals:

  • Exercise Check-in - every 4 days, starting on Day 5 after the "RTM: Include in Patient Care Plan” is answered
  • Pain Check-in - every 7 days, starting on Day 7 after the "RTM: Include in Patient Care Plan” is answered
  • Goals Check-in -  every 14 days, starting on Day 6 after the "RTM: Include in Patient Care Plan” is answered
  • General Well-Being Check-in -  every 8 days, starting on Day 8 after the "RTM: Include in Patient Care Plan” is answered

The patient’s provider will be able to review the results of the patient’s Questionnaires, and can take action based on the results. For example, if the patient indicates on an Exercise Check-in Questionnaire that the assigned Exercises are too easy, the provider can assign the patient different or additional Exercises. 

Additionally, a notification will be visible to providers when patients provide certain responses on the Exercise Check-In Questionnaire and the Goals Check-In Questionnaire. An image of a stop sign will be visible on the Exercise Check-In Questionnaire if the patient responds that the exercises are too easy, too hard, or other. An image of a stop sign will be visible on the Goals Check-In Questionnaire if the patient responds with neutral, disagree, or strongly disagree to the question of whether they think they are making progress towards their goals.

RTM Education

Patients will be assigned pieces of Education over the course of 8 weeks. These pieces of Education are:

  • Importance of Your Home Exercise Program
  • Importance of Monitoring Your Progress
  • You Are Not Alone
  • Why Keet

RTM Monitoring Time Count

The RTM Monitoring Time Count Questionnaire will be assigned to a patient’s Care Plan as part of the RTM Global Workflow. This Questionnaire is only visible to the provider, and is meant to help providers track the amount of time they spend remotely monitoring each patient. The information that providers input in this Questionnaire will feed into the RTM Billing Dashboard. In order to utilize this part of the dashboard, the RTM Monitoring Time Count Questionnaire must be completed. When completing this Questionnaire, enter the date that the monitoring occurred, and the number of minutes spent monitoring the patient.
