A duly authorized representative may give consent on behalf of a group, but not for an individual MIPS-eligible clinician reporting as an individual. If your organization intends to report only as “Group” via the Keet Outcomes QCDR you may have a duly authorized representative give consent on behalf of the group for Keet Health to submit the group’s data to CMS.
If your organization chooses to participate as a Group only, the organization's performance data (non-individually identifiable) will be submitted to CMS.
If you choose to participate as a Group only, individual consent from providers is not needed, and all of your organization's performance data will be included in your final submission.
If you choose to participate as Individuals only, only those with consent provided will be able to have data submitted for them.
If you choose to participate as both Group and Individuals, all of your organization's performance data will be included in your final submission, and only the individual data of providers who have provided consent will be submitted individually. (You may choose to report as both, “Individual(s)” and “Group” and only have a subset of your group report individually while all clinicians will be reported as “Group”. For more information, reference this article).If a provider left your organization and no longer has access to Keet, but wishes to provide consent for the 2021 MIPS Performance Year, they may provide consent by filling out this form.
The Clinician Consent Dashboard pulls providers in based on the role of Practitioner. With that, profiles for non-MIPS eligible practitioners may be pulling into the dashboard. The presence of those profiles will not impact reporting, and only practitioners with a valid NPI entered into Keet, and individual consent provided will be submitted individually to CMS.
