Keet Outcomes Dashboard Troubleshooting

The Keet Outcomes Dashboard provides valuable data regarding your patient's progression throughout their duration of care. If you notice that the information on your dashboard is not reflecting what you would anticipate seeing, review the required fields and FAQ sections below: 

Required Fields

In order for data to accurately populate in the Keet Outcomes Dashboard certain information must be included in Keet. Review the sections below to ensure that all the necessary fields are completed: 

Patient

If the patient is present in Keet with the External ID included in their profile and qualifies to be reported on, they will appear on the dashboard.

Episode of Care
  • A status of Active or Finished
  • Supervising Provider
  • An Episode of Care Start Date
  • MIPS Qualification (toggle)
  • External Identifier (Case ID from your EMR)
Care Plan
  • A status of Active or Finished
  • Care Plan Start Date
  • Location (Clinic) 

Note

If there are completed outcomes forms (PROs) on the Care Plan, the status should match the Episode of Care status-- both should be in an active or finished state. If a Care Plan is deactivated but the associated Episode of Care is active, then any associated PROs from that Care Plan would be removed and it may appear as if data is missing from the dashboard. 

 

Troubleshooting / FAQ

What impact does Episode of Care status have on reporting? 
Active; these episodes will pull through to the outcomes dashboard along with the PROs on associated Active or Finished care plans
Canceled; these episodes will NOT be carried over into the outcomes dashboard 
Suspended; these episodes will NOT be carried over into the outcomes dashboard 
Entered in Error; these episodes will NOT be carried over into the outcomes dashboard 
Finished; these episodes will pull through to the outcomes dashboard along with the PROs on associated Active or Finished care plans
*If there is no start date, this will not be pulled through to the dashboard
What impact does Care Plan status have on reporting? 
Active; the PROs on these care plans will pull through to the outcomes dashboard
Canceled; the PROs on these care plans will NOT pull through to the outcomes dashboard
Suspended; the PROs on these care plans will NOT pull through to the outcomes dashboard
Entered in Error; the PROs on these care plans will NOT pull through to the outcomes dashboard
Finished; the PROs on these care plans will pull through to the outcomes dashboard
*If a care plan is deactivated but the associated EOC is active, then any associated pros from that care plan would be removed and make the EOC look like it's missing data. If there are completed outcomes forms on the care plan, the status should match the EOC status. i.e. both should be in an active or finished state. 
How does the MIPS disqualified tab on the Episodes of Care impact reporting?
If an EOC is marked as disqualified, the data would not make it into the individual numbers or process reports BUT it would show in the performance reports in regards to the eligible/ineligible population.
How does the classification of 'Other' on the Episodes of Care impact reporting?
Episodes of Care that are marked as "Other" in the Primary Classification will be filtered out of the dashboard completely. 
How do EOCs that have been migrated impact reporting?
PROs that were completed in 2021 will show in the Titled/Named Episode of Care for users that have their EMR connected to Keet. Non-integrated users will see all of the relevant PROs under the Migrated Episode of Care
 
 

Non-Integrated Accounts 

If you are an account that does not have your EMR linked to Keet and are seeing issues/missing data in your Reports please ensure that the following information is being included: 

  • Encounters are logged on the Episode of Care. An Arrived Encounter is required
  • External IDs are included on the patient and Episode of Care
  • Ensure IDs for Practitioners and Locations are entered
