Add a New Questionnaire of the Same Type

If you want to add a subsequent questionnaire of the same type to a patient's profile you can complete that action by following these steps. For example, if the patient has completed a Stress Index Questionnaire and you want to assign another Stress Index Questionnaire. 

1. Click Patients in the upper navigation bar
   
2. Search for the patient and select the patient's profile
3. Click the appropriate questionnaire to open it
4. Click Add Another Questionnaire below the list of current questionnaires
   A new questionnaire is then added beneath the last completed
