Assign a Questionnaire to a Patient

If you need to assign a questionnaire survey to a patient's profile be sure an Episode of Care is created along with a Care Plan. 


  1. Click Patients in the upper navigation barmceclip0.png

  2. Search for the patient you want to assign the questionnaire to
  3. Locate the Care Plan to which you want to add the questionnaire
  4. Click Add Activity within the Care Plan

  5. You can search for keywords or Click the Global, Account, or Organization boxes, as appropriate, to view the desired questionnaire 

  6. Scroll down and click the + sign in the appropriate questionnaire tile
