Send a Care Plan to a Patient

When adding or editing a patient's Care Plan it is recommended to resend the care plan to the patient to ensure that they are reviewing the most up to date content. 

1. Click Patients in the upper navigation bar
   
   
2. Search for the patient you want to assign the questionnaire to
3. Locate the Care Plan you want to send to the patient

       4. Click the Email Care Plan icon to send the updated Care Plan to the patient
